Who this policy covers

This policy describes how Athlos Wellness, LLC, doing business as Athlos Health ("Athlos Health," "we," "us"), handles information collected through www.athloshealth.com. It applies to this website only. It does not describe how we handle protected health information for people who become patients of the clinic — that is governed separately by our Notice of Privacy Practices, which you will receive at the time you establish care.

The Longevity Assessment

How it works, and where your answers live

The assessment at /longevity-score/ asks about symptoms across six areas — hormones, energy and sleep, metabolic health, cognition, physical capacity, and mood. Every part of the scoring happens in your browser. Your answers are not sent to us as you go, they are not saved to an account, and if you close the tab before the end they are gone. There is nothing for us to keep unless you decide to send it.

At the end you are shown your results. Beneath them is an optional form. If you complete that form and tick the consent box, we receive:

• your name, email address and, if you give it, your phone number;
• your age and biological sex, as you entered them;
• your overall Athlos Longevity Score and your six pillar scores; and
• your individual answer to each of the twenty-four questions.

We ask for the full answer set deliberately. Dr. Traughber reads it before your consultation so that the appointment starts from what you have already told us rather than from a blank page. If you would rather not send your answers, you are welcome to call us instead and skip the form entirely — the assessment is useful to you whether or not you send it to us.

What the assessment is not

It is an educational tool that records how you feel and scores it consistently. It is not a diagnosis, not a clinical screening test, and not a laboratory measurement of anything. It does not estimate your biological age, and it produces no age figure of any kind — no questionnaire can measure that. Sending us your results does not create a doctor–patient relationship and is not a request for treatment.

Please keep clinical detail out of it

Please do not add detailed medical history, diagnoses, or medication lists to the form. Ordinary email and web forms are not secure or encrypted channels, and we cannot guarantee their confidentiality in transit. Send us enough to start a conversation, and we will move anything clinical to a secure channel or a phone call.

What else we collect

Information you choose to send us

Aside from the assessment, you can reach us by calling the phone number or clicking the email address. If you do, we receive whatever you choose to tell us — typically your name, your phone number or email address, and whatever you say about why you are contacting us. We keep that so we can respond to you and, if you become a patient, so we have a record of how you first reached us.

Information collected automatically

This site is hosted by Vercel. Like essentially every web host, Vercel's servers record standard technical request logs, which can include your IP address, browser type and version, the pages you requested, and the date and time of the request. These logs exist for security, abuse prevention and basic reliability. We do not use them to build a profile of you, and we do not attempt to identify individuals from them.

Analytics — only with your consent

We use Google Analytics 4 to understand, in aggregate, how visitors find and use this site, so we can improve it. It is off by default. On your first visit you will see a banner; analytics loads only if you choose "Accept," and if you choose "Decline" — or simply ignore the banner — no analytics cookies are set and no analytics data is collected. We remember your choice on your own device so we do not have to ask every time.

If you consent, Google Analytics sets first-party cookies to measure things like pages viewed, how you arrived, general location (city or region, not your precise location), and device and browser type. We have enabled IP anonymization. You can withdraw consent at any time by clearing this site's data in your browser, which resets the banner so you can choose again.

Advertising measurement — only with your consent

We advertise this clinic, and we use conversion measurement from Google to understand which advertisements lead people to contact us. Like analytics, it is off by default and loads only if you choose "Accept" on the banner.

What is reported is deliberately minimal. When someone submits the assessment form, Google is told that an enquiry occurred on this website. It is not told who you are, what you scored, how you answered any question, your email address, or which areas the assessment flagged. We do not use enhanced conversions or advanced matching, we do not upload customer lists, and we do not place your score or any answer into a web address where an analytics or advertising tool could read it.

We do not build advertising profiles, we do not sell personal information, and we do not "share" it for cross-context behavioral advertising as California law uses that term.

This website does not carry a Meta pixel, and carries no other advertising, remarketing or cross-site tracking technology beyond what is described above.

Third parties that receive data because of this site

Vercel hosts and serves the site, and processes the request logs described above.

Web3Forms delivers the assessment form to our inbox. When you submit the form, its contents pass through Web3Forms' servers on the way to us. They act as our processor for that delivery and do not use the contents for their own purposes. If you would prefer your answers not to pass through a third party at all, call us instead of using the form.

Google Analytics (Google LLC) provides the analytics described above, but only after you consent. When it is active, Google processes that data as our service provider under the Google Ads Data Processing Terms; IP addresses are anonymized.

Google Fonts supplies the two typefaces this site uses. Your browser requests those font files directly from Google's servers, which means Google receives your IP address and browser information as part of that request. We do not control and do not receive that data.

Google Workspace provides our email. If you email us, or send us your assessment, your message is stored in our Google Workspace mailboxes.

We do not sell your personal information, and we do not share it with third parties for their own marketing.

How long we keep things

Assessment submissions and enquiries. If you go on to become a patient, your submission becomes part of how we came to care for you and is retained on the schedule described in our Notice of Privacy Practices. If you do not become a patient, we delete your submission and any related correspondence within twelve months of your last contact with us.

Server logs are retained on our host's ordinary schedule, and Google Analytics data on the retention schedule configured in our Analytics property. Records created once you become a patient are retained according to California medical record retention requirements, which are longer and are addressed in our Notice of Privacy Practices rather than here.

Your rights in California

If you are a California resident, the California Consumer Privacy Act, as amended by the California Privacy Rights Act, gives you the right to know what personal information we have collected about you and where it came from, to request a copy of it, to request that we delete it, to request that we correct it if it is inaccurate, and to be free from discrimination for exercising any of these rights.

Sensitive personal information. Answers you send us through the Longevity Assessment describe how you feel physically and mentally, and California law treats information of that kind as sensitive. We collect it only when you choose to send it, we use it only for the purpose we described when you sent it — preparing for and arranging your consultation — and we do not use or disclose it for any other purpose. Because we limit its use to that purpose already, there is nothing further to opt out of, but you may ask us to delete it at any time and we will.

We do not sell or share personal information for cross-context behavioral advertising.

To make a request, call us at (310) 933-6863 or email info@athloshealth.com with "Privacy Request" in the subject line. We will verify your identity before acting on a request, which for most people means confirming details we already hold. You may use an authorized agent; we will ask for proof of their authority.

Children

This site is intended for adults. Our services are for adults, and the assessment is written for people aged 25 and over. We do not knowingly collect personal information from anyone under 18. If you believe a child has sent us information, contact us and we will delete it.

Security

The site is served over HTTPS. We take reasonable measures to protect the information we hold, but no method of transmission or storage is perfectly secure, and we cannot guarantee absolute security — which is the reason for the caution about clinical detail above.

Links to other sites

This site may link to other websites, including our partner facility. We are not responsible for the privacy practices of sites we do not operate, and we encourage you to read their policies.

Changes to this policy

We may update this policy as the site changes. When we do, we will revise the "last updated" date at the top. Material changes will be described here rather than made silently.
